Because of the volume of information required to enter 4-H members in the WVU database we will be collecting the Enrollment forms and Health forms at the Extension office. Please fill out the Enrollment form for 4-H Members AND Cloverbuds.
We will mail you a copy of each members paperwork along with a completed Summary sheet as soon as possible after November 1st.
It is our hope that this will allow for faster entry into the database and less paperwork for our leaders.
